We highly recommend not handling lenses at all prior to patient application because we have painstakingly prepared them for that.
To be properly inspected in our quality control check at the end of production, the lenses must be pristinely clean. This allows us to detect even the smallest of imperfections under high magnification. Once a lens passes QC, it is not only pristinely clean, it remains untouched by human hands and is immersed in conditioning solution for shipping. This ensures that the lens is in optimal condition for direct application to the patient. Handling the lenses and cleaning them prior to patient application often results in what you are seeing. The handling and cleaning induced a non-wetting situation on the lens surface.
There is an in-office routine that most often reverses this situation and I have attached a document to this email outlining the procedure. If non-wetting persists after this routine, using some Biotrue solution and aggressively working it into the lens surface is often effective.
I also wanted to mention that we strongly recommend Hydra-PEG for all scleral lenses since they are high Dk materials that are prone to developing non-wetting issues easily, even after weeks of being worn.
